The Social Work Department at the Richmond County Health Department offer several Case Management programs to assist the community.

Pregnancy Care Management (PCM)

Pregnancy Care Management (formerly known as Maternity Care Coordination) provides case management for Medicaid eligible women. Pregnancy Care Management (PCM) is a statewide program in N.C. promoting healthy mothers and healthy babies. This program provides care management services for high risk women during pregnancy and for two months after delivery. Pregnancy Care Managers can assist with referrals to community resources, applications for assistance/resources, and prenatal/parenting/newborn care education.

Care Coordination for Children (CC4C)

Care Coordination for Children (previously known as Child Service Coordination) is a free service that ensures that children are raised in healthy, safe environments by supporting families and connecting them with vital services. CC4C care managers can help with finding medical care, transportation, childcare and /or financial aid. They can also provide you with information about a wide variety of family oriented resources.

CAP Program

CAP (Community Alternatives Program) is a program sponsored by the State of North Carolina as an alternative to nursing home placement. A patient must meet the eligibility criteria for a skilled nursing facility and qualify for North Carolina Medicaid. Richmond County Health Department provides CAP Case Management Services for both Adults and Children. Once on the CAP Program a person can receive additional benefits which they cannot receive through traditional Medicaid. CAP exists to supplement, rather than replace, formal and informal services and family support already available to an individual. Richmond County Health Department understands that people are happier and healthier in the comfort of their own home.
